He was one of several Autobots killed in the 1986 movie, a move that led to many distraught fans writing letters to the studio addressed to "Optimus Prime" asking for him to be brought back. Up to that point, Peter Cullen had absolutely no idea how popular the character was as he had never been given fan letters and the character returned for a single episode called "Dark Awakening". However, fan letters continued to arrive and he was brought back again in the 2-part episode "The Return of Optimus Prime".

It happened during the second movie,But here's what happened

during the forest scene Optimus was fighting 4 or 5 Decepticons and when he was finish with what he though the last Decepticon Megatron came behind him and stabbed him in the back and through his chest.

First movie: Yes he dose die. Sam shoves the Allspark into his spark and he dies.

Second Movie: He is brought back to life by a shard of the Allspark. He loses an arm but he dose not die.

Third movie: Yes, Optimus ripes his head off.

Once Optimus is revived the fallen attacks him again so Jetfire allows the Autobots to disassemble him and combine with Prime (killing Jetfire in the process) with this new ability of flight, new weapons (jetfires engines make a sort of shock-wave firing thingy) and the fact that he is a Prime the Fallen never stood a chance even with Megatron by his side. Prime rips the Fallens face off then mercilessly stabs the Fallen with his own staff to deal the killing blow.

Yes, he does, in a forest battle with Megatron starscream, and Grindor. He got stabbed in the chest, by Megatron. Sam ( shia) he needs to get the Matrix, so the fallen don't get it and operate the machine. Sam thinks, if the matrix can do that why cant he bring back Optimus. in the last minutes of the movie (20 minutes approx) Optimus comes back because Sam puts the matrix into Optimus's chest. He then combines with Jetfire, Jolt and ratchet helps him in-order to that. Optimus becomes this big robot. he goes on top of the pyramid, fights Megatron and the Fallen, kills the Fallen. Meagatron leaves along with Starscream
