n the beginning when Banner first turned to Hulk, he was gray. He also only turned to Hulk at nights. Later when he started to control his hulking out with a gamma machine, he was green and the transformation was triggered by adrenalin.

Why doesn't the hulk turn into banner during planet hulk and world war hulk?
Quite frankly, he didn't want to. At this point in the Hulks life, Banner was in such a state he didn't want to transform back from the Hulk. And, Banner did make appearances in both Planet Hulk and World War Hulk
