OK, so first put your gameboy game that you want to migrate from in one slot of the DS then, put the DS game that you want to migrate to in the other. Go onto your Pearl game and choose Migrate from (whatever), you have to pick 6 Pokemon to migrate, so make sure one of them is Jirachi. Then press ok. Go to the Pal Park and catch them.
(You must have beaten the Elite Four and the Pal park opeed for you to do this.)

There is no such thing. I restarted the game 3 times to find it but you cannot get a jirachi on diamond/pearl. You need to migrate one from a GBA pak.
