He hit him with the Avada Kedavra curse when they were moving Harry.
Voldemort was aiming for the Harry beside him (who was actually Mundungus) because he thought that the real Harry would be with the strongest auror. However Mundungus disapparated and the curse hit Moody.
That's the Spanish translation for Madeye Moody, the nickname of Alastor Moody.
Lord Voldemort didn't actually know or care who Cedric Diggory was. Cedric ended up in the graveyard by mistake and Voldemort didn't want to leave any loose ends, so ordered Wormtail to "kill the spare".
Voldemort didn't just want to kill Muggle-borns in the second book. He had always wanted to kill them. The reason is because his father, who was a Muggle-born, left Voldemort's mother pregnant. Voldemort's mother was a witch, but after falling in love with Voldemort's father, she gave up all her powers. After that, when Voldemort realised that he was a wizard, he looked down to Muggle-borns.
It was necessary for the plotline: Harry had to hunt the horcruxes and confront Voldemort alone (at least without any help from adults). The same goes for Dumbledore and Moody's death and Lupin's wife getting pregnant.
yes
Professor "Madeye" Moody was captured during the Goblet of Fire and was taken by Bartemus Crouch Jr. so that he could impersonate Moody and get Potter to Voldemort in the cemetery.
That's the Spanish translation for Madeye Moody, the nickname of Alastor Moody.

Alastor Moody was killed by Lord Voldemort. Moody had been hit in the face with a Killing Curse.
Moody was killed during a battle with Death Eaters. Voldemort killed him personally while moving Harry from the Dursley's house. He was aiming for the Mundungus, who was disguised as Harry beside Moody, but Mundungus disapparated and the curse hit Moody instead. This happened in The Deathly Hallows, Chapter 5, The Fallen Warrior. He died in the diversion the Order of the Phoenix created. One of the Death Eaters killed him. (7th book, Deathly Hallows)
Voldemort didn't kill him, nor did anybody else.
Lord Voldemort killed Alastor Moody because he was helping to transport Harry Potter to the Burrow. To make it harder for Voldemort to capture Harry, six people took Polyjuice potion with Harry's hair in it - which meant there were seven Harry Potter's and the Death Eaters and Voldemort wouldn't know which was the real one. Each of the Potter's would fly with an adult guard to a safe house and would take a portkey to the Burrow. When they took off the Death Eaters attacked and Voldemort thought that the real Harry would be with the most experienced Auror so he went for Moody and his Harry, who was really Mundungus Fletcher. Mundungus was reluctant to take part in the plan anyway and as a killing curse was sent towards him he disapparated, the curse then hit Moody.
no. he almost dies, but then pretends to be dead while Voldemort tortures him and eventually kills Voldemort.
Alastor "Mad-Eye" Moody is killed by Lord Voldemort; Voldemort knocks him off his broom with a curse after he is abandoned by Mundungus Fletcher, who is acting as a decoy, and Moody plummets to his death during the Seven Potters chapter of Harry Potter and the Deathly Hallows.
