The District 4 boy died in the bloodbath at the Cornucopia though the details of *how* he died are unknown. The girl was killed when Katniss dropped the tracker-jacker nest on the Careers. She died of tracker-jacker stings.

Glimmer (District 1) dies from numerous tracker jacket stings from Katniss dropping the nest on the Careers when they were waiting under a tree she was in to kill her. Marvel (District 1) also died from Katniss, she shot him with and arrow after killing her little ally. Clove, (District 2) was killed by Thresh who in the book threw a rock at her skull which dented it, but in the movie the picked up her body and slammed her against the Cornacopia, which cracked open her skull. Cato (District 2) died from the mutations made from the Game makers that were made from the fallen tributes. District 3's male and female tribute both died from the blood bath that happened at the Cornacopia. So did 4's male and female tributes. District 5's male tribute was also killed at the bloodbath, while the female tribute, FoxFace, died from eating Night lock berried after trying to be sneaky and stealing them after Peeta picked them, but Katniss said that her body was emaciated, so she would've died from natural causes soon anyways. 6 , 7 , 8 , 9 , and 10's tributes both male and female tributes died at the bloodbath. District 11's female tribute, Rue, died from a spear in the stomach, from Marvel. Thresh, the male tribute, died from fighting with Cato for a long while. Both Katniss and Peeta had near death experiences, Katniss with the tracker jackets, and Clove tried to kill her by cutting her up with knives at the feast at the coracopia. Peeta was sliced on his upper leg very badly after saving Katniss and Cato was the one who cut him, and he got blood poisoning. There were attempting double suicide, at the end, but Claudius Templesmith goes on the intercom, announcing Katniss Everdeen and Peeta Mellark the victors of the 74th Annual Hunger Games.
