I found it in Cerulean Cave. It's the same cave where you find Mewtwo but I think I had to use the Dowsing Machine to find it. (there are NUMEROUS hidden items to find there). Don't get this cave confused with Mt Moon though. This cave can be accessed only after collecting all 16 badges. To access it you need to travel north on the bridge from Cerulean City to the Cape, but instead of heading east at the top of the bridge, head west as far as you can then go south until you reach the water. Use Surf and follow the waterway until you see the cave. If you go there BEFORE getting all 16 badges, a man will be blocking the entrance and won't let you in.
Get it in the Pokeathlon for like 2500 points.
