You'll need to find HM07 which contains Waterfall inside this cave. To reach the item you need to navigate the ice on the bottom floor carefully so that you place your character at the ladder on the bottom of the screen. Grab HM07 and teach it to a pokemon that can learn Waterfall. Use it to go up the waterfall at the start of the cave and go to the ladder. You should find Team Rocket Grunts and you'll need to take care of them to save Lorelei. Helping Lorelei will let you go into Dotted Hole on Six Island.
