To get to Island 4, you must first get the national Pokedex from Professor Oak. This requires you to:
Beat the Elite 4
Have 60 Pokemon in your Pokedex.
You must then return to Island 1, Celio will send you to get the Ruby, return with the Ruby (If you have headed to Mt Ember on Island 1 you will see the rockets, that's where the Ruby is). He will then ask you to get the Sapphire and upgrade your Tri-Pass.
