Certain cards place a counter called an A-Counter on other cards. You represent this with a coin, glass bead, or anything similar. Thus a card with one of these, becomes a card with an A-Counter.
Since other kinds of counters exist in the game, it's up to the players to keep track of which is which.

No. The Yu-gi-oh card maker is just for fun, and not officially affiliated with Konami (the company that owns Yu-gi-oh). You can just make cards that look like it but you can not actually print it like a real yu-gi-oh card.
