Go to the select menu go down to the book, open it up and turn the page once to the right and there you have (button sequence : esc; down; ctrl or enter; right; ctrl or enter)

You can't really skip a level in Tomb Raider: Legend without completing the game first (of course you could cheat if you're on the PC version uses a Save Game). With completed the game you'll unlock a replayability section in which you can replay any level within the game. You cannot, to my knowledge, skip a level in Tomb Raider: Legend - but you could in the older games by 'exploding' Lara (something I'd love to return in future games!).

To save load and exit you press escape: esc and to see health items weapons and notebook press tab (above caps lock)

There is an option in the menu, at the very top, I think. I think it also autosaves so no worries.

There is no particular order you need to play the levels in, but if you want to be able to use save games from the internet the preferred order is Nevada, Pacific Islands and then London.
