Specifically the passing of his brother River and Johnny Cash's brother. I suspect it was a mental hospital or psych ward due to the nature of the hospitalization. I think it could very well be a possibility of depression and post-traumatic-stress disorder. During the scene where Cash learns of his brother's fatal accident in the Saw Mill factory, Phoenix started to bang his head against the bed post. He said he was getting into the role. I just know he was sent to the hospital for observation. This was 2005 and years before "I'm Still Here"

He left acting and schooling at 16 to travel South America with his father. At this time, he changed his name from Leaf back to Joaquin. On returning to Florida he was part of a few different bands, one of which was with his brother River. He was present at the scene of River's death in 1993, two days after his 19th birthday; he made the call to the emergency services. He returned to acting a year later in Gus Van Sants 'To Die For'(1995). Van Sant had previously directed River in 'My Own Private Idaho' (1991) and Rain in 'Even Cowgirls Get The Blues' (1993) He received an Oscar nomination for 'Gladiator' (2001), making he and River the first (and only, so far) brothers in history to be nominated from Oscars in the acting category. He was nominated for an Oscar again for 'Walk The Line' in 2005. He won a Golden Globe that year. He checked himself into rehab for alcohol issues in 2005. In 2008 he stated to E! News that he was retiring from acting to pursue a music career and began making the 'documentary' 'I'm Still Here; The Lost Year of Joaquin Phoenix' In 2009 he played a handful of rap shows and appeared on David Letterman. His birth name was Joaquin Raphael Bottom; His parents changed their last name to Phoenix by deed poll in 1979 when Joaquin was five years old. He has a scar on his top lip which he was born with. Damaged his shoulder in a bike accident when he was 16. He spoke fluent Spanish as a young child.
