The first mention of Neville losing the passwords was in Chapter 12: the patronus; page 249. he was explaining to sir cadogan that he wrote the whole week's passwords on a piece of paper but lost them. it was in chapter thirteen:gryffindor versus ravenclaw page 268, where we saw the effect of what had happened; sirius black found the passwords and thus gained entrance to the commonroom.
