Episode 54 titled "Beyond the Inferno." Envy commits suicide by tearing out and destroying his own philosopher's stone.
Maes Hughes was killed by Envy in episode 25 of the original anime, episode 10 of Brotherhood, and in chapter 15 of the manga. For the record, Envy is an evil bastard, and I was happy when it died in CoS. It also killed Edward, but he came back thanks to Al.
Maes Hughes is alive in Full Metal Alchemist: Brotherhood because Brotherhood is a remake of the series, not a sequel, so everybody who died in the original series will most likely be alive but die in this series. I hope that answered your question!
NO WAY! Envy can't die he is a Homunculus and obviously they are immortal! I hope FMA: Brotherhood becomes English Dub here, in America! BIG SPOILER Actually, they're not immortal, just difficult to kill. Destroying their core destroys them. And, unfortunately, Envy dies in chapter 95, which is episode 54 in FMA: Brotherhood
In the 2003 anime, Lust died in episode 47. In Brotherhood (2009 anime) Lust died in episode 19.
Don't worry, she doesn't.

It depends on whether you're talking about FMA or FMA brotherhood. However, in both instances the show starts after her death. However, there are flashbacks relating to her death in the first two episodes of the first series and near the end of the series when Edward confronts Sloth, the homunculus he and Al created when trying to bring their mother back to life. In the second series, their mother's death is mentioned but flashbacks relating to her are scarse. In one of the middle episodes (around 25-ish, if I remember correctly) Honenheim visits Pinako and learns about his wife's death. There is a brief flashback there. There is also a flashback around episode 53 when Ed meets Honenheim in a hobo village near central.
