No. About half way through the movie they say that they are giving him drugs. These drugs allow them to put any story they want into his head and he'll believe it, just like the story that he killed his wife and he was on the island for 2 years. The story they tell him at the end was a complete lie so that they could say that he was crazy and they could perform brain surgery on him in the lighthouse(last picture in the movie).
Its very hard to tell because there are supporting answers for both sides. For example, when they first show up on the island the guards are nervous, it could be because he is a patient. However an example that he is not crazy is that they say that he is being given drugs. It is impossible to truly tell if he is crazy or if he is sane, either way, he was right about the brain surgery.
