The original Sherlock Holmes never had a "lady friend." Irene Adler was one he referred to as "the woman" out of respect for her intellect and the fact that she bested him in ONE case. Watson states in the story however, that he never felt anything akin to love for her.

She appears in the story A Scandal in Bohemia and is the only woman who outwitted Sherlock Holmes and he ever after only referred to her as "The Woman." Irene Adler, who was referred to by Holmes as "the woman", appears in "A Scandal in Bohemia".

The Conon Doyle (real)Sherlock Holmes never had a girlfriend. Many people can't settle with that, so they make Irene Adler his girlfriend, even though it is plainly stated that, (to quote) 'it is not that Holmes ever felt anything akin to love for Mss. Irene Adler.' I am not criticizing you, but I am criticizing the people who give Holmes a girlfriend in movies and books! It really makes me mad, why can't they just let the un-sexually feeling Holmes alone. It makes for much more moral books, and I like it! But if you do want a slight love story, read The Sign of Four, Watson gets a girl and it's really cute!
