i think that, because of what Ozai said to Zuko during the eclipse, she killed Firelord Azulon in order to save Zuko, this would explain why is was banished.

Why was zukos mom banished in avatar?
No one can be entierly sure, but from the way Ozai put it in the Day of Black Sun, she killed Azulon to save Zuko and put Ozai on the throne rather then Iroh. She may also have (my personal guess) forged something that made Ozai Fire Lord, but again, this last part may not be true. It's never explained why Ozai is fire lord, so my guess is that she also had something to do with it, which was also considered treason.

As for Ursa being Zuko's girlfriend instead of his mother? That's just a bunch of bull. :P --- Actually in (Season 2) Book 2, in episode 7: Zuko Alone. This is when Zuko proclaimed to the village: "I am Zuko, son of Fire Lord Ozai, and Princess Ursa..." this confirmed that Zuko's mother's names is in fact Ursa, and was the first time it was mentioned.
