Great question. I have been dying to know this for years, but it remains unknown. It is not disclosed in the books, nor has J.K. Rowling ever mentioned it in any of her interviews. My personal guess would be that his boggart takes the form of Lily's dead body, as her death was by far the most traumatic thing that ever happened to him.
In the third book it was failing all her exams.
It turns into Professor McGonagall telling her she failed all of her exams. (LOL)!

Although tricky, the easiest method for banishing a Boggart is with the "Riddikulus" charm, transforming it into something humorous (instead of horrifying). The resulting transfiguration will kill the boggart.Adaptation of "ridiculous". Spell specification:Causes an item, creature or person to take on a humorous appearance of the spell caster's choosing. They are more bothersome than bogies. They are usually recognized due to the unusual number of mishaps that occur while a boggart is in the house. No way is known in regards to the elimination of the boggart, except leaving the house, although sometimes this doesn't even work, as boggarts are sometimes transported with household items.

A boggart is a shape-shifting creature that takes on the form of its intended victim's worst fear. It generally likes to hide in dark, enclosed places. In Harry Potter and the Prisoner of Azkaban, Professor Remus Lupin teaches his students in Defence Against the Dark Arts to approach a boggart in groups of two or more, so that the boggart will have difficulty in choosing which one to frighten. A common wizard's defense against a boggart is to point a wand at the boggart and say "Riddikulus" while thinking of something very funny; this charm puts out a protective shield so that the boggart can be held at bay, or manuevered away.
