What does it mean when some yu-gi-oh equip card's work if they're sent to the graveyard 'while equipped to a monster'?

It mean that to trigger, they had to be sent to the graveyard while they were equipping a monster. That means if they were discarded from hand, or destroyed while face down, or destroyed because the equipped monster was turned face-down or was removed from the field, then they will not get their effect. But if it was equipped to a monster, and an effect destroyed it (or destroyed it and the monster simultaneously) then that effect will be applied.

But if you wanted to summon another Lv4 monster, you cannot simply make room by sending a monster from the field to the graveyard, you can only do so by effect or game mechanic, like when it is destroyed by battle.

Yes, you can.You can special summon Synchro Monsters from the Graveyard, but they need to be properly Synchro Summoned first...Just like a Fusion monster. You see, if you properly synchro Summoned a monster and your opponent used "Lightning Vortex", you can simply use a "Monster Reborn" or any other card to recover it; since it was properly summoned. Although, if it is sent directly from your Extra Deck to the Graveyard by a card effect, you can say "bye-bye" to that card for the rest of the duel.
