In the movie, Wade Wilson (pre-Deadpool) was subject to many kinds of torture which is supposed to give him a mutation. Wade Wilson (pre-Deadpool) got his mutation from being put in a chamber that gives him enough oxygen to breathe but not much oxygen where he feels like he is suffocating.
