during the events on mustafar during episode three (Revenge of the Sith), he was scarred beyond repair because he was caught on fire from a nearby lava river. the fire did too much damage to his internal organs, hence why he walks around in his life support suit, and never is seen outside of it (minus his unhelmeted head in a specific scene of episode five).
