during the events on mustafar during episode three (Revenge of the Sith), he was scarred beyond repair because he was caught on fire from a nearby lava river. the fire did too much damage to his internal organs, hence why he walks around in his life support suit, and never is seen outside of it (minus his unhelmeted head in a specific scene of episode five).

Are congestive heart failure and diastolic heart failure the same?

Congestive heart failure is a buildup of fluid due to a failure of the heart to maintain a normal forward flow. It is a pump problem. Symptoms my include swelling of the soft tissues of the extremities, particularly the lower extremities, shortness of breath from buildup of fluid in the lungs, inability to lie down due to shortness of breath, decreased appetite, and exercise intolerance. An enlarged heart may be seen on chest xray in a patient with congestive heart failure, but other conditions may cause an enlarged heart as well, including chronic hypertension.

Did Marlon Brando die of cancer?

His cause of death was listed as pulmonary fibrosis. It was noted he also had congestive heart failure, diabetes, and liver cancer. Like Errol Flynn, it was a race between Brando's heart, lungs, and liver on which would give out first. Amazingly, Brando lived to age 80 despite numerous health issues.
