Yes, he did die creating the philosophers stone whom is al because al was about to explode because of another alchemist scar turned him into the philosopher's stone. But in brotherhood he doesn't die.
Scar had his arm ripped off and he implanted the power of his other arm into Alphonse. He was then shot by one of the Soldiers and fell into the gigantic transmutation circle he created, fusing both him, and the other soldiers into the Philosipher's Stone that became a part of Al's body... If memory serves me right.

His brother gave it to him before he died.
In the series, they never reveal his actual name. At the end, Olivier asks him his real name but Scar wants to give up whatever name he secretly had and the nickname "Scar" because he says he's giving up his past lives or whatever, and Olivier just ends up calling him Ishvalan.

he is killed before the start of the series, and it's the reason scar has that arm with tattoos. you see his death in volume 15 of the manga.
The name is Lust, not Lusk. And she is Scar's brother's ex-lover. Edit: She is not Scar's brother's ex-lover, she is the homunculus that was created as a result of Scar's brother's failed attempt at bringing his dead lover back to life. She has the same appearance as Scar's brother's lover, though, much like Sloth looks like Trisha.

Yes! Funimation recently announced that "Fullmetal Alchemist: Brotherhood" will be dubbed and released sometime in Spring 2010. The main cast is almost entirely the same, with the only noteable changes being Scar and Alphonse receiving new voice actors (since the voice actor who originally played Scar now does live action work in LA and the actor who originally played Alphonse has experienced puberty). Though he is not going to be voicing Alphonse, Aaron Dismuke will be playing another role in the series, according to Funimation. Also on the 14 of Feb. adultswim is going to show the first episode
you can't make a scar. it happens like when you fall or something like that and if you pick on the wound it will leave a scar.
