In Chapter 6 of Harry Potter and the Prisoner of Azkaban, Professor Trelawney is teaching Divination class. She instructs the class to drink tea and interpret the leftover tealeaves.
Professor Trelawney finds a Grim (stating "it's one of the darkest omens in our world"), the black dog who haunts churchyards and is seen as a death-omen, in Harry's cup. This finding alarms everyone except for Hermione, who is skeptical and somewhat flippant about the whole situation.
Sirius Black is mistaken for The Grim because Sirius is an unregistered Animagus. He assumes the form of a dog resembling the description of a Grim.
This scene could be seen as foreshadowing of his death in 1996.

In Harry Potter and the Prisoner of Azkaban, Sybill Trelawney, the divination teacher associates Harry's tea leaves with the Grim which she calls, a black dog who haunts churchyards (Sirus Black was mistaken as Grim due to his animagus form).PetigrewHere, Professor refers to Peter Pettigrew (servant of Voldemort) who was actually responsible for the deaths of Harry's Parents and was the best friend of James Potter and Sirus Black. (It is for this reason that Sirus Black was being wrongly held responsible for the deaths of Harry's parents): He will return tonight! He who betrayed his friends - whose heart rots with murder!.And in these lines, Innocent blood shall be shed and servant and master shall be reunited once more!, she refers to the Buckbeak, the hippogriff who attacked Malfoy, the servant of Black. He is to be put on trial and executed. Harry and Hermione rescue him from execution. They then save Black and send him away to freedom on Buckbeak's back. Thus Servant and Master are reunited.

He is considered to look similar to Gregg Sulkin who plays Mason on Wizards of Waverly Place. Also, he is SOMETIMES mistaken as Daniel Radcliffe who plays Harry Potter in the Harry Potter series.
