Kreacher kept the locket at Grimnauld place and in Harry Potter and the Order of the Phoenix it was thrown out when the house was being cleaned. Kreacher then rescued it and kept it in his cupboard.
When Sirius died, Mundungus Fletcher broke into the house and stole several items; including the necklace. He tried to sell it but was stopped by Dolores Umbridge and forced to give it to her for free.
Harry, Ron and Hermione then broke into the Ministry to steal it from her and it was later destroyed by Ron.
