Superman
Superman's human name given to him by Martha and Johnathan Kent is Clark Kent. Superman's Kryptonian name given to him by Jor-El and Lara Lor-Van is Kal-El.
Kal-El on his home planet of Krypton. Clark Kent by his adoptive family.
That's sort of a trick question. When he was born on the planet Krypton, his parents named him Kal-e (the "eEl" at the end of his name is the family name, as in his father Jor-el's name). When he came to earth as a small child, the people who found him were Jonathan and Martha Kent. They adopted him and named him Clark after Martha's family, the Clarks. So you see, his name was BOTH Clark Kent AND Kal-el.

Jerry Siegel and Joe Shuster decided to make that his first name because of the actor Clark Gable. Jonathan and Martha agreed on that to be his first name after they found and adopted him because that was his adoptive mother's middle name.
