Who kills who in The Hunger Games?

In the 74th Hunger Games, the first 11 people are killed at the cornucopia. It really doesn't say who killed who there although we know Clove (from distict 2) killed one boy. Then later that night the carerrs killed a girl who started a fire and fell asleep. After that Katniss dropped a nest of tracker jackers that killed Glimmer (district 1) and the girl from district 4. After the tracker jacker venom wears off Katniss hears a cannon go off and we find out the boy with the bad leg was killed, although we're not sure how he died. After that Katniss and Rue team up and blow up the carerrs food, causing Cato to become angry and kill the boy from district 3. Then Katniss goes looking for Rue but finds her tied up in a neat and gets killed after Marvel (from distict 1) throws a spear at her. After killing Rue, Marvel is killed by Katniss. Then at the Feast, Clove is kiled by Thresh. Then it's implied that Thresh is killed by Cato to get revenge on him for killing Clove. This leaves Katniss, Peeta, Foxface, and Cato. Foxface is killed after she accidently eats some poisonous berries that Peeta accidently gathers. Then the final death is Cato who is almost killed by the pack of mutts that attack him, but he's finally finished off after Katniss shots an arrow through his head. After that Katniss and Peeta almost kill themselves in order to keep each other alive since the Gamemakers tried to force each other to fight to the death.
