He was not really Mad-Eye Mood, he was Barty Crouch, Jr., a Death Eater who was drinking Polyjuice Potion to look like Moody.

Putting Harry's name in the Goblet of Fire was part of the plan to get Harry involved in the Triwizard Tournament so that he would end up touching the Triwizard Cup which Crouch had turned into a portkey taking Harry to a graveyard where Voldemort was coming back to life and planned on killing Harry. (That didn't end up working out).

He confounded the Goblet into thinking four schools competed, and then entered Harry's name under the fourth school, as he was the only option it was inevitable he would be selected.

When the staff are discussing how Harry's name could have come out of the Goblet, Barty (as Moody) suggests that this was the course of events carried out by an unknown person - who he implies is Karkaroof.
