Yes she does. In an image we can see Ahsoka dueling with Dar Vader. Whether she survives that duel I do not know. If you want to see the image follow this link and scroll down to the image named 'Now my enemy'.

Yes
First of all, no one knows what Ahsoka's fate is yet. So who knows if she is dead before Order 66 or not? We'll have to wait until Season 3 ends in order to find out.
If Fives is around during Order 66, that is still yet to be revealed
It is estimated that around 200 Jedi survived Order 66, as they were able to escape or were off-world when the order was issued. These survivors went into hiding to avoid detection by the Empire.

Order 66 was the codename for the command that all the Clone Troopers should immediately turn on their Jedi commanders and kill them. Palpatine gave the order and told the Clone Commanders it was because the Jedi were exacting their plot to take control of the Republic, but it was actually Palpatine that was taking control.
No she did not. But she did lead a clone army as a general during the Siege of Mandalore at the end of the Clone Wars. That was the same place she was when Order 66 was issued and she escaped and survived. To find out more on this backstory, read the novel "Star Wars: Ahsoka" written by E.K. Johnston
Pablo-Jill did not actually die. All the sites are wrong, but if you see the entire Clone Wars series (especially the old one) you will see that Grievous disarmed and struck him down, but he only wounded him. When his body was thrown out the ship into space for all Republican Forces to see he landed on the outskirts of Coruscant and survived. During Order 66 he was with master Eeth Koth on a mission on the planted Cato Neimoidia, and they both fought together to kill all the clones. While they both survived, they're whereabouts after Order 66 are unknown, although it is assumed that they both eventually died, possibly of old age. -Michelangelo
