In the anime, Sailor Galaxia fought during the Sailor Wars against Chaos, and in order to save the galaxy, sealed Chaos within herself. The presence of Chaos within her began corrupting her, though, so in order to protect it, she sent it out of her body to somewhere in the Galaxy where it would be taken care of. This Star Seed took the form of Chibi Chibi, and later Sailor Chibi Chibi.
In the manga, Sailor Galaxia had a more "evil" beginning--she was born on a planet she regarded as "trash," and roamed the Galaxy, seeking the ultimate power. She encountered a hooded individual (with more than a passing resemblance to Wiseman/Death Phantom--a form of Chaos, and one that Galaxia was aware was one of the most powerful in the galaxy) that told her of the Sagittarius Zero Star, the birthplace of all stars in the galaxy (and the center of the galaxy). Galaxia ended up setting up her base of operations there, at the Galaxy Cauldron, and recruiting minions to help her acquire more and more Star Seeds. Those who killed "true" Sailor Senshi (with genuine Star Seeds, also known as Sailor Crystals) could gain the powers of the defeated Senshi by utilizing bracelets given to them by Galaxia. A "true" Senshi who gave up her Sailor Crystal could also survive in a physical form with the use of these bracelets.
If Sailor Galaxia succeeded in making Sailor Moon distraught, she would become vulnerable to Chaos. If Sailor Moon was overtaken by Chaos, the two immense powers would cancel each other out and destroy one another, leaving Galaxia to get their Sailor Crystals and then become the strongest Sailor Senshi in the galaxy.

How did Sailor Pluto die?
In episode 197, Pluto and Saturn were the last two senshi to get their Star Seeds taken. The four Outer senshi decided to go and challenge Galaxia themselves as to not endanger Sailor Moon... or whatever other reason they might have had. When Galaxia was about to take their Star Seeds, Uranus and Neptune decided to become her minions. Their Star Seeds were taken, and the infamous Galaxia bracelets appeared on their wrists. Once Pluto and Saturn realized that their comrades were serious about getting their Star Seeds, they allowed them to be taken when Uranus and Neptune fired. Right after Pluto and Saturn's star seeds got taken, Moon and the Starlights ran in. We have a very sad departing as the two outer senshi fade away. =(

Does Chibi Chibi Moon come from a Sailor Senshi's Scepter?
No. In the animated series, Chibi Chibi (Chibi Chibi Moon when she transforms) is the physical manifestation of the "Light of Hope," and Sailor Galaxia's Star Seed. She transforms into a sword to enable Princess Serenity to battle Sailor Galaxia when she is fully transformed by Chaos, but Princess Serenity chooses not to use the sword to fight Galaxia. Galaxia shatters the sword with her own, causing it to revert back into the seemingly-lifeless body of Sailor Chibi Chibi Moon. However, after Sailor Moon has defeated Galaxia and Chaos within her, Chibi Chibi reappears and thanks Sailor Moon.In the manga, Chibi Chibi's origins are different: she is actually the disguised version of Sailor Cosmos. She was from a devastated future where Chaos fully gained control over Galaxia and became Sailor Chaos, destroying everything that Cosmos held dear. Even if the war between Chaos and Cosmos ended, nothing would be salvageable, so Cosmos went into the past to guide Sailor Moon to destroy Chaos and the Galaxy Cauldron.In the final chapters of the manga, Sailor Cosmos revealed that she had a closer connection to Sailor Moon, but it is still not entirely clear what that connection is.
