It's written into the storyline to provide some comic relief. As for the characters, to my knowledge there is no singular event that caused the friction between McCoy and Spock, it's just that they each approach life very differently. Spock makes decisions in a calm, cool manner utilizing his highly developed analytical skills and logical methodology while McCoy acts more instinctively and applies his medical knowledge and skills based on more humanistic goals.
Leonard Nimoy has remained as Spock ever since Star Trek began. There was never a Spock before Nimoy. Although Nimoy has always played Spock, before production Deforest Kelly (Doctor McCoy) was cast as Spock but was later considered better suited to play Doctor McCoy. Also, in the May 2009 movie; Nimoy will only be playing the older Spock since he is turning 78 on March 26th, 2009. Zachary Quinto will be playing the younger Spock in this movie. (If you do not understand how there can be a younger && an older Spock in the same movie, please ask a question about it!)

After his eyesight returned and Spock is once more the bridge, McCoy whispers to Kirk, "Don't let Spock know that I said he was the best damned first officer in the fleet."
