An irreversible reaction is one in which the entire amount of a reactant or all of them are converted into products. They proceed only in the forward direction and an arrow pointing to the products is placed between reactants and products in the reaction equation. A chemical reaction becomes irreversible when one of the products leaves the homogeneous system where the reaction takes place. e.g. suppose A and B are the reactants, and C and D are the products. The reaction takes place in liquid phase. If one of the products precipitates (appears as an insoluble solid), or is a liquid that is not miscible with the reactant liquid phase, or is a gas that comes out of solution, then we have an irreversible reaction.
