It is formed when gas in the volcano vent causes an explosions which shatter the hot (sticky magma of a stratovolcanoe) flinging it high into the air from where it descends back to earth as a fine powder made of sharp (microscopic) shards which are bad to breath in.

The ash can fall a long way form the volcano (down wind) and can accumulate to many meters thick. The ash gradually compacts and solidifies into rock but before this happens, if there is a heavy rain storm the ash can form a thick mud which flows like a river (caled a lahar) down the volcano. Lahars can be very distructive.
