If you mean accident with nuclear weapons, the US codename for that is Broken Arrow.
One example is the incident at Palomares Spain, where a US Bomber carrying 4 hydrogen bombs collided with a refueling tanker. Both planes crashed and the unarmed bombes fell on the Spanish coast. 2 landed inland with physical damage but no explosion, 1 landed inland with chemical explosion in primary on impact, and 1 landed in the water and remained lost for a few months until located by divers.
Another example iis the Thule Greenland accident, where a B-52 with unarmed hydrogen bombs had an onboard electrical fire and the crew bailed out. The plane then crashed on the ice and at least one bomb had a chemical explosion in its primary.
Some examples of nuclear energy:A fission reaction at a nuclear power plant provides enough energy to give electricity to large cities. The fusion reaction in the sun provides our planet with all of the energy it needs for living organisms to survive. An uncontrolled fission reaction provides the destructive force of a nuclear bomb.source: softschools.com
A nuclear accident is the unintended release of nuclear radiation into the environment, such as damage to a nuclear reactor or to a nuclear weapon (plane crash carrying nuclear bombs, etc). The two basic forms of nuclear weapons are fission and fusion weapons.
Currently we do not have any good solutions for disposing of the nuclear waste, which will remain dangerously radioactive for thousands of years. Additionally, accidents and nuclear power plants can be catastrophic and can potentially make an area uninhabitable.

Timeline of the Fukushima nuclear accidents happened in 2011.
So far nuclear energy should be called bane. The waste disposal problems and reactor accidents outweigh its lack of emissions.
Three major nuclear accidents; namely:Three Miles Islands nuclear accident, USA, March 1979Chernobyl nuclear accident, Ukraine (former Soviet Union), April 1986Fukushima nuclear accident, Japan, March 2011
Between 1945 and 1987, there were 285 nuclear reactor accidents, injuring over 1,550 people and killing 64.
Thankfully there haven't been many nuclear accidents, however when they do happen they can be severe the worst nuclear accident/disaster was the explosion of reactor No.4 at the Chernobyl Nuclear power plant in 1986 on April the 26th.
Yes, there have been accidents at nuclear power plants, with the most well-known being the Chernobyl nuclear disaster in 1986 and the Fukushima Daiichi nuclear disaster in 2011. These accidents have led to significant environmental and health consequences in the affected regions.

Some surrounding problems with nuclear power include the risk of accidents and radioactive waste disposal. Accidents like meltdowns can have severe consequences for human health and the environment. Additionally, finding long-term solutions for storing radioactive waste safely is a challenge.

Nuclear fusion and nuclear fission are processes that involve nuclear reactions but are not examples of radioactive decay. Chemical reactions, such as burning wood, do not involve nuclear processes and are also not examples of radioactive decay.
