Depending on the state or county of the deceased, it could be on file with the Clerk of The Court - the Health Department - The Bureau of VItal Statistics - etc. Best advice is to call the information number for one of these agencies and ask. It is not an uncommon question and I'm sure that they would give you the answer.

This largely depends on where the patient died. If it's a well-known teaching hospital, especially one that has its own mortuary services division, it will provide the receiving funeral home with the death certificate signed by the doctor. If an autopsy was performed, the medical examiner or the coroner will provide the death certificate by the ME or Coroner. Finally, if the patient died at home or a community hospital, the funeral home will fill out the preliminary information on the death certificate given by a family member. It will then take the certificate to the physician who will sign the death certificate. The funeral director will then take the death certificate to the vital statistics Bureau of the health department, which will scan in the death certificate prepared by the preparing the entity and create the official copy, which will have the Department of Health's seal and registry information, making the death certificate official.
