Narcissists rarely fight physically because they are afraid of getting hurt. They primarily engage in verbal abuse and "gaslighting". However, if they are really threatened, they may burst out physically, but expect pushing, throwing, and "girl'type" fighting. A narcissisist will rarely engage in a full-fledged fist fight. Now, be warned. Even though they may not beat someone up, they are not beyond plotting killing someone if their relationship or identity has been threatened. So the answer is yes, they can get violent, but no, not in a typical way.
Yes they are.
They always emotionally abuse them, because narcissists lack empathy. Some physically abuse. It depends on the specific case.
Yes
No. Narcissists can be only children, middle, large families. No correlation.
If you've been exposed to narcissistic abuse in childhood, you're more likely to end up with narcissistic partners if you haven't worked through your abuse. Adult Children of Narcissists (ACONs) have a number of issues to resolve.

NO -- Of course they can, anybody can abuse anybody.
Yes there are children, usually teens, who abuse their parents both verbally and physically.
Substance abuse in children is alcohol; street drugs; stealing prescription drugs from their parents or smoking.
Psychiatric therapy, but unfortunately narcissists are so into themselves (selfish, egomaniacal to the point of verbal abuse and sometimes physical abuse) that they feel they don't need help and most seldom will go for treatment.
