Any smoke/smoking can trigger emphysema. Emphysema is linked to an inherited protein deficiency. Macrophages (defence cells) in the air sacs in the lungs 'eat' the inhaled particulates in smoke. A lack of the protein Alpha-1 Antitrypsin (AAT) allows the microphages to also destroy the colagen and elastin that are essential for lung expansion and contraction. 1 in 500 people have AAT deficiency in varying degrees. Therefore 1 in 500 could, through inhaling particulate smoke, develop emphysema.
