Symptoms include a dull, throbbing cramping in the lower abdomen that may radiate to the lower back and thighs.
There is a small pimple which seems to be right above the dull pain and perhaps an engrown hair or some other solution or the result of a fall on back side of head in the past. This could also be a result of nerve or muscle stress on the back of the head.

Is the pain sharp? If so, go to the ER RIGHT AWAY. You could have a serious kidney infection. If the pain is dull or feels better/relieved when you stretch it's probably a strained muscle.
Sounds like a kidney.
