It is a planter wart. Yes, they are VERY painful! I myself am trying to find a cure for it w/o going to the podiatrist. My son had this and the doc had to cut them out! OUCH!!! Mine is about the size of a quarter now and it's very painful!

yes - any break in the skin could introduce bacteria that could lead to an infection. Always wash and disinfect, after removing the splinter, if you can. If you can't get the splinter out, wash and disinfect anyway.

I am not sure how you could have left it so long to be treated it must have been incredibly painful. You may find that the infection has tracked back up through the bladder and ureters tothe lidneys where it may casue permanent damage, and/or it may settle into the bladder walls causing a painful condition called interstitial cystiitis which is extremely resistant to treatment.
