The most important factor if you're really serious about this is the feel. Silicone, which is what the majority of implants are made of, will feel harder than the fat-tissue that breasts are normally composed of. The feel may be less relevant if the implant is a salt-water implant.

Secondly you may see an unusually sharply defined transition from chest to the breast itself, although if the surgery is done as professionally as it should be, this may not be the case.
