How do you test a biceps reflex?

The triceps muscle reflex can be performed as follows:

1. The patient's upper arm is abducted (extended out to the side - away from the body)

2. The forearm is allowed to hang toward the floor.

3. The tendons which attach the triceps muscle to the elbow joint should be facing basically upward (toward the ceiling) at this point. These tendons can be identified by pressing near the elbow joint and feeling for the "rubbery" cord-like structures.

4. Place the finger firmly against one of these tendons, then strike the finger digit with the reflex hammer.

5. Watch for the hanging hand to move away from the body slightly. This is the reflex.

Put the thumb of your nondominant hand over the biceps brachii tendon in the crook of the patient's flexed elbow, cupping the rest of your hand around the olecranon. Hit your own thumb with the reflex hammer and watch for a contraction.

Place your thumb over the antecubital fossa (inside of elbow) of the patient. Then tap the reflex hammer onto your thumb and observe the reflex in their biceps.
