Doctor's normally send messages straight to the other doctor's office that you're being referred to. Once they receive the referral electronically, that office should call you and schedule an appointment with you. Your doctor should tell you the name of the doctor you're being referred to. If you want more information, you can ask.

To write a referral letter to Social Services, you need to have factual information that can be supported by hard evidence. If you feel that a child's life is at risk, you must say so in the letter but you need to be able to back up the information that you are passing on. Show your concern for the child without telling the Social Worker her job.
