There are many tiny blood vessels in the lungs called capillaries and these have thin membranes through which oxygen can flow from the lungs into the blood. They also exchange carbon dioxide which you exhale.

The heart carries carbon dioxide rich blood to the lungs where it is expelled and oxygen is absorbed into the blood when you inhale and that oxygen rich blood goes back to the heart along a different artery than the one carrying the carbon dioxide rich blood.
