How is emphysema spread?

It's when you smoke. Smoking causes your air sacs ( littles sacs in your lungs to get bigger, decreacing the amont of surface area in your lungs) and making you unable to breathe and if you smoke too much you get Lung Cancer and you'll die.

Emphysema is not a contagious disease, so it is not spread between people. It is caused by the degeneration and destruction of the alveoli and bronchioles deep in the lungs, usually by smoking although severe Asthma or pneumonia can cause this as well.

Emphysema is caused by lung irritants - even things like chronic or repeated exposure to chemicals (not just smoking or 2nd hand smoke). As the lungs become irritated, it affects the alveloi which collapse or flatten. The irritant also causes increased phlegm, which further impedes gas exchange.
