Usually devices to stretch a tight foreskin aren't necessary. Most men can accomplish stretching with fingers and a corticosteroid cream (betamethaone 0.05%, available by prescription). However, it sounds like your foreskin is really tight, way too tight to get a finger inside. First of all, be sure the use the device very gently. It should just cause gentle stretching. If you have any pain you're stretching it too hard. Any blood or tearing means you're doing it way too hard. You should get a corticosteroid cream from your doctor, for example betamethasone, and apply that to the opening of the foreskin. If possible you should try using your fingers to gently stretch the opening of the foreskin. This might take several weeks or a month, but you should be able to stretch the opening enough to use the device. In the event you are unable to stretch the opening of your foreskin, you have three options. The first would be to opt not to treat the phimosis. If the phimosis isn't causing any problems like pain during sexual activity, then this is a viable option. The second option is a conservative operation to enlarge the opening of the foreskin with performing a circumcision. Often this can be accomplished by an incision in the frenar band (where the inner and outer foreskin meet). Other times the frenar band may be removed but the vast majority of the foreskin left in place. This is a conservative type of circumcision, where most of the foreskin is left in place. The final option is a complete circumcision, which most men which to avoid since it removes the entire foreskin and can result in the desensitization of the penis.
