This depends on whether the illness is episodic or not. Take depression, for example. An individual can either have episodes of depression where they show symptoms with full recovery and periods of normalcy between or they can show symptoms all the time (with periods where the symptoms are worse than others).

Whether they show them all the time or not will depends on the type of mental illness and possibly the subset.
