When a male patient goes to the doctor after the age of 50, the doctor will perform a prostate exam. A male can not usually touch their own prostate, and generally not know what abnormalities they are looking for.
Need to know the patient's age, and whether the patient is new or established.
Need to know what your question is.

Not anywhere that I know of. You do generally need a post-graduate degree, though.
The EOB (Explanation of Benefits) is what explains Medicare's payments and denials. Sometimes it is referred to as an EOP (Explanation of Payment). This document will show all items filed on a particular claim for a particular provider. It will show which items were covered or noncovered and why, which items were denied and why, and which items were paid. It will also show the patient's responsibility as far as deductible and coinsurance goes. If the patient has a Medicare supplement or just a secondary plan and Medicare is aware who you have chosen, they will "crossover" the claim to the secondary. This means they will automatically send a notification to the secondary payor to let them know how much Medicare allowed (the total amount the provider should receive from Medicare, other insurance companies and the patient), and how much is being left to the patient/secondary.
Oh, dude, let me break it down for you. Beta prostate and prostate perform are like distant cousins at a family reunion - they're related, but not exactly the same. Beta prostate is a supplement that contains beta-sitosterol, while prostate perform is a different supplement with its own blend of ingredients. So, like, they're both for prostate health, but they're not identical twins, you know what I mean?
Oh, prostate health is so important, friend! You can learn more about it from your doctor, who can provide personalized advice and guidance. We need to observe the symptoms of the disease and age of the patient while administering medication. One should also be careful about the expiry date of the medicine before giving it to the patient.
There may be some evidence that lycopene may help to prevent prostate cancer, but as of yet, there is insufficient data to know for sure.
