In Diabetes mellitus, glucose can't enter the body's cells from the blood, since insulin is low (or ignored). Blood glucose begins to rise. When blood glucose gets high enough, it passes the 'renal threshold' for sugar. (In humans this is between 180 and 220 mg/dL.) The kidneys are then obliged to filter out glucose and excrete it with the urine.
At this point, the urine becomes too concentrated and osmosis pulls extra water into the urine from the body, leading to the polyuria symptom. The resulting dehydration makes the victim thirsty -- this is the polydipsia system.
The sugar being wasted by the kidneys means the body is not nourished, even with enough food intake. This leads to excess hunger or polyphagia.

I don't think that there are four (4) polys.....However the three (3) that I know of are: Polyurea....Polyphagia....& Polydipsia! Poly = Many urea = urination phagia = eating dipsia = drinking. Poly (many) urea (urination)
Diagnosis of diabetes can be made with an oral glucose challenge (if the diagnosis is uncertain), by a fasting blood glucose of > 125mg/dL, or a random blood glucose of > 200mg/dL, when accompanied by any symptoms concerning for diabetes (polydipsia, polyuria, polyphagia, or weight loss). The glycosylated hemoglobin (also called HgbA1c) will also be elevated, but this is not currenlty used to diagnose diabetes.
Patient may come with a classic triad of polydipsia, polyphagia and polyuria. That drinking extra water, eating extra food and urinating a lot. He may come with diabetic coma. You detect diabetes by doing the blood glucose level.
