The smoke does because when you are around a bunch of smoke it makes it harder to breathe which causes you to cough so you can get all the bad stuff out of your system and breathe normally again.

A smokers lung will be filled with tar making it harder to breath, it also causes the heart to pump alot faster. You will notice that the smoker will cough because the lungs are trying to get the harmful chemicals out. The lungs may eventually develop cancer tumors.
