There probably wasn't one specific disease. Some common ones, however, were dysentery and malaria.
Other factors that caused the colonists to die were starvation, drought, exhaustion, and attacks from American Indians. At one point in the colony's history, the Powhatan colony initiaed a period of forced starvation. this caused over 70% of the population to die from starvation and cold during the "Starving Time" of 1609-1610.
