A simple NLP technique of linking & dissociating a phobia may be helpful. For example, instead of trying to reduce your phobia of Spiders by throwing you into a spider tank (full immersion), I may link the image of a spider to a more innocuous image (eg. a spider riding on the back of a horse), and then have you visualise the horse riding away, watching the spider get smaller and smaller until your fear also reduces in size. There are many good techniques, this is just one example.
