Too many calories consumed with too few calories used up in physical activity. In other words, choosing the wrong foods (e.g., that are too high in fat, are the wrong types of fats, not enough lean meats and proteins, not the right kind of carbohydrates, and typically portions are two or three times the appropriate size) and doing too little physical activity with sedentary jobs, too much time in front of the TV or computer screen, not enough time outside walking and playing, etc.
